PO-04-188 PREMATURE VENTRICULAR COMPLEXES: ASSESSING BURDEN DENSITY IN A LARGE NATIONAL COHORT POPULATION TO BETTER DEFINE OPTIMAL ECG MONITORING DURATION
نویسندگان
چکیده
PVC burden frequency has been shown to correlate with the incidence of PVC-mediated cardiomyopathy. A single center study 107 patients wearing Zio monitors indicated that measurement for 24-48 hours may not reflect total burden. This aims assess density, and significance long-term monitoring in a large national cohort. The objective this is establish optimal duration ECG required accurately characterize severity other significant factors. Patients ≥5%, age ≥18 ≥13-day wear period were analyzed between June 2011 April 2022. Patient demographics reported, mean absolute error was calculated at baseline (<10 sec), 24 hours, 48 daily to14 days. Burden end considered true value measured described each subinterval (mean, SD, median, IQR). accuracy evaluated across durations 10% (low),15% (moderate) 20% (high) thresholds. There 106,705 analyzed, 33.6% females an average 67.3±15.4 years old. density more frequent males than females, 66:34 respectively those older 65 age. (<10sec), 7 days, 13 days (Figure1). When comparing densities lower higher data showed decreased from 2.78% 3.97% 1.27% 1.75% Furthermore, categorization 15% threshold increased 89.2% 95.1% Most common concerning arrhythmias detected do show important delineating trend consider (Table1). In cohort, reduction through 14 underscores importance ambulatory extends prior results. Increased least 7-10 associated improved by level. <20% benefit longer compared >20% population. Concomitant arrythmias including VT Afib patient further emphasized closer early detection.Tabled 1Table1Total Device Count (%)Low (5%-10%)* n=59,702Moderate(10%-20%)* n=34,605High (>20%)* n=12,398Overall* n=106,705VT37,858 (63.4)23,799 (68.8)9,251 (74.6)70,908 (66.5)AFIB9,021 (15.1)4,123 (11.9)1,119 (9.0)14,263 (13.4)SVT46,068 (77.2)26,712 (77.2)9,495 (76.6)82,275 (77.1)AVB1,467 (2.5)671 (1.9)181 (1.5)2,319 (2.2)Pause (≥3 sec)3,479 (5.8)1,682 (4.9)466 (3.8)5,627 (5.3) Open table new tab

Classification of Premature Ventricular Contraction in ECG
Cardiac arrhythmia is one of the most important indicators of heart disease. Premature ventricular contractions (PVCs) are a common form of cardiac arrhythmia caused by ectopic heartbeats. The detection of PVCs by means of ECG (electrocardiogram) signals is important for the prediction of possible heart failure. This study focuses on the classification of PVC heartbeats from ECG signals and, in...

متن کاملVentricular premature complexes in prognosis of angina.
We studied the prognostic role of ventricular premature complexes occurring during 1 hour of electrocardiographic monitoring of 416 men with effort angina who had never had myocardial infarction, and compared mortality over 5 years with that of 1739 men with infarction before first observation. Multivariate analyses of survival identified the presence of ventricular premature complexes in 1 hou...
متن کاملPremature ventricular complexes as a trigger for ventricular fibrillation.
INTRODUCTION AND OBJECTIVES The mechanisms that trigger ventricular fibrillation (VF) are poorly understood. The aim of this study was to analyze the initiation of VF in electrograms stored in implantable cardioverter-defibrillators (ICDs). METHODS We analyzed ICD electrograms from patients who had suffered at least one episode of VF. RESULTS Of 250 patients with ICDs, 13 (10 male and 3 fem...
متن کاملEvaluation and management of premature ventricular complexes.
Premature ventricular complexes (PVCs) are a common cause of palpitations. Related symptoms include difficulty breathing, chest pain, fatigue, and dizziness. PVCs are also commonly detected incidentally on electrocardiography, outpatient ambulatory monitoring, and inpatient telemetry. Treatment goals include palliating symptoms, restoring cardiac function if affected, and preventing progression...
